Bizarrely, the grad school keeps a Word template, but not a LaTeX template.  There is a old, unmaintained, "no guarantees" LaTeX style file at the grad school Web site.

Over the years, a number of UTCS Ph.D. candidates have maintained a LaTeX template, which is attached here.

Note that the Vita section generated by the **utthesis-2.0ame.tar** template is optional, according to the Grad School format rules. Consequently, so is the completion of the **\thesisauthoraddress** command.
